Anyone ever find throw doesn't throw?

I’m seeing occasionally if I add a throw into some code, it doesn’t actually throw.

also sometimes I’m seeing changes to source code not being picked up.

this is mostly with 5.2.6.60, but also with 5.2.7-latest
